To save time, you can print these forms out and complete them at home. <\/p>\r\n\r\n<a style=\"display:flex; justify-content:center;\" href=\"https:\/\/mthoodent.com\/resources\/patient-forms\/\" class=\"button\">Find Your Forms Here<\/a>\r\n\r\n<p>In addition to the completed forms, you will need to bring\r\nthe following with you to your first visit:<\/p>\r\n\r\n\r\n\r\n<ul><li>Insurance card.<\/li><li>Referral from your primary care physician (if\r\nrequired by your insurance provider).<\/li><li>Your payment\/co-payment.<\/li><li>A list of current prescriptions and\/or\r\nover-the-counter medications you are taking, including dose and frequency.<\/li><\/ul>\r\n\r\n\r\n\r\n<p>In order to make the most out of your first visit, here are\r\nsome additional recommendations: <\/p>\r\n\r\n\r\n\r\n<ul><li>Write down your questions prior to the\r\nappointment. This will help to make sure you address all your concerns with\r\nyour doctor.<\/li><li>Write down any symptoms you may have. Try to be\r\nas specific as possible. <\/li><li>Bring an objective family member or friend to\r\nyour appointment. They can help you answer questions and provide additional\r\ninformation the doctor may need.<\/li><li>Wear comfortable clothes.
